Attached is the two page Sarah Bush Lincoln Dental Services form that will be sent out this week to all students.
The completed form will need to be turned into the school office by Friday, August 29th in order to guarantee the student will be seen in the spring. Forms can be submitted after that date, but it is not always a guarantee they will be seen.
